CONSENT AND RELEASE

 I hereby authorize National Women in Roofing, Inc. (“NWIR”) to record my participation, appearance, likeness, and voice on any digital, analog or other device or storage medium, including DVD, CD, video tape, audio tape, or photograph (the “Materials”).  I understand and agree that any photograph or video using my likeness will become the property of NWIR and will not be returned. All intellectual property rights that are associated with the Materials are the sole property of NWIR.

I acknowledge that my participation with NWIR is voluntary and I will receive no financial compensation.

I hereby unconditionally and irrevocably authorize NWIR to copy, alter, edit, publish, exhibit or distribute photos or videos depicting my likeness for purposes of publicizing NWIR’s purpose or for any other related, lawful purpose.

I acknowledge that the Materials may be used for the purpose of telecasting, advertising, illustration, public relations, or publications on behalf of NWIR.

I waive the right to approve or inspect the finished product, including electronic or written copies, wherein my likeness appears. In addition, I waive any right to royalties or other compensation related to the use of my likeness.

I hereby hold harmless and release and forever discharge NWIR from all claims, and causes of action which I, my heirs, executors, representatives, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am at least 18 years of age and I am competent to contact in my own name. I have read this release before submitting or agreeing to photos or video recording and I fully understand the meaning, contents, and impact of this release.
